Abstract

This study tested the hypothesis that sildenafil daily for 1 year can cure vascular erectile dysfunction (ED). In a prospective, randomized controlled trial, patients were assigned to two groups: group I, 50 mg of sildenafil daily at bedtime; group II, 50 to 100 mg of sildenafil on demand. The primary efficacy measures were International Index of Erectile Function domain score and peak systolic velocity (PSV) of penile arteries after treatment and following the nonmedicated periods. Treatment with 50 mg daily sildenafil for 1 year resulted in enduring normalization of erectile function in 59% of patients and a PSV of the cavernous arteries improved from 29.4 ± 9.8 cm/sec to 38.9 ± 10.3 cm/sec (P < 0.05). Sildenafil taken daily for 1 year resulted in ED regression and may be a useful tool for curing ED.
