Zusammenfassung
In diesem Beitrag werden philosophische Grundlagen und zentrale Annahmen der kognitiven Anteile kognitiver Verhaltenstherapien dargestellt und das Vorgehen bei der praktischen Umsetzung dieser Anteile beschrieben. Als sitzungsübergreifende Charakteristika werden der sokratische Dialog und die Binnenstrukturierung einzelner Sitzungen hervorgehoben. Die weitere Beschreibung einzelner Bausteine der Kognitionsveränderung und ihrer Rahmenbedingungen folgt dem Vorgehen der Arbeitsgruppe um Beck, da dieses empirisch am besten evaluiert worden ist. Es gibt überzeugende Belege für die Wirksamkeit kognitiver Verhaltenstherapie bei behandlungsbedürftigen Patienten eines weiten Störungsspektrums. Sie werden bei Depression und Angststörungen als Therapien der ersten Wahl in internationalen und nationalen Leitlinien herausgestellt. Neben der Klärung theoretisch und klinisch wichtiger Forschungsfragen werden die größten zukünftigen Herausforderungen in der Implementierung eines manualgetreuen Vorgehens in den klinischen Alltag ambulanter und stationärer Therapien gesehen sowie in der Weiterentwicklung spezifischer Strategien zur Aufrechterhaltung der akut erreichbaren Therapieerfolge.
