To the editor:

—We read the case report and review of hydroxychloroquine overdose by Marquardt and Albertson1 with interest. Hydroxychloroquine overdose is rare, but important, particularly with its increasing use for many autoimmune and rheumatological disorders. We would like to describe our unit's experience over the last 13 years.

There were 7,245 cases of deliberate self-poisoning that presented to the Hunter Area Toxicology Service (HATS) for the period January 13, 1987 to June 30, 2001. All cases of

HCQ blocks sodium channels and potassium human-ether-a-go-go-related gene (hERG) channels, slowing cardiac intraventricular conduction and lowering the seizure threshold (16). Additional potassium efflux pump blockade leads to hypokalemia by inhibiting potassium egress from the intracellular space, which can be exacerbated by vomiting, bicarbonate-containing fluids, and epinephrine (15,21). The delayed cardiac myocyte repolarization and resulting prolonged refractory period prolongs the QRS and the QTc intervals, which may increase the risk of fatal dysrhythmias including torsades de pointes.

Hydroxychloroquine was initially synthesized from chloroquine in 1946 to decrease the incidence of associated toxicities [9]. Chloroquine is estimated to be several times more toxic [10]; however, deaths have been reported from hydroxychloroquine overdose [11,12]. Published case reports of hydroxychloroquine toxicity are found as early as 1960 [13].

In chronically treated patients, monitoring of HCQ concentrations helps clinicians in identifying adherence and has been shown to contribute to predicting HCQ treatment efficacy [2,6,7]. Despite its wide use and recognized safety, accidental and deliberate overdoses have been reported resulting in severe poisonings and fatalities [8–18]. However, due to insufficient data, usefulness of HCQ concentrations in overdoses remains unclear.
