Abstract

We report the first case of priapism associated with the use of sildenafil citrate. The patient was a 28-year-old man with mild erectile dysfunction after penile trauma who self-treated with a 100-mg dose of sildenafil citrate, resulting in low-flow priapism. After aspiration and intracorporal injection of alpha-agonists, the priapism resolved. The patient’s erectile function returned to his prepriapism baseline.

Comment

To our knowledge, this is the first reported case of sildenafil citrate-associated priapism. Sildenafil citrate-related priapism is exceedingly rare. The package insert states that episodes of sildenafil citrate-associated priapism “have been reported infrequently since market approval of Viagra.”1 We are uncertain as to the impact of the prior penile trauma in our patient. The patient’s youth, near normal erectile function, and relatively high dose may have contributed to the occurrence of

Some medications have been associated with ischemic priapism, most commonly vasoactive injectable erectile dysfunction drugs that have produced an excessive erection response (31). With the advent of oral pharmacologic drugs in the class of phosphodiesterase type 5 (PDE5) inhibitors such as sildenafil, tadalafil, vardenafil and avanafil for the treatment of erectile dysfunction, precautionary concerns were raised about their possible risk for inducing priapism (35). However, observational and scientific studies have not borne out this concern, and experts in the field have well countered this presumption (36).

Of note, 27% of men with an incidence of priapism in the latter analysis had been using concomitant medications that might have contributed to the priapic episodes.56 Few other individual case reports of PDE5I-induced priapism have been published, some of which were confounded by the concomitant use of opiates and intracavernosal injections.66–70 Recently, emerging data have even suggested a potential role for daily PDE5I therapy for the suppression of priapic episodes in men with stuttering priapism, a condition in which PDE5 dysregulation is believed to be contributory.71

Tadalafil has significant association with myalgia. Priapism theoretically may be caused by any ED drug; however, the rate of priapism with PDE5i is close to zero.47 Of course, the risk of priapism may be amplified in men who combine PDE5i’s with other drugs (prescription or recreational).

Serious ophthalmic side-effects may occur, generally after longer exposure to the drug: branch retinal artery occlusion (Tripathi and O’Donnell, 2000), central serous chorioretinopathy (Allibhai et al., 2004), anterior and posterior ischemic optic neuropathy (Pomeranz and Bhavsar, 2005; Su et al., 2008), acute angle-closure glaucoma (Ramasamy et al., 2007), optic atrophy (Sowka et al., 2007), and stepwise decline in visual field (Pepin and Pitha-Rowe, 2008). Priapism (Sur and Kane, 2000), tonic-clonic seizures (Gilad et al., 2002), vestibular dysfunction (Hamzavi et al., 2002), and amnesia and aggressive behavior (Milman and Arnold, 2002) have also been reported. Intracerebral hemorrhage (McGee et al., 2005), sudden sensorineural hearing loss (Snodgrass et al., 2010) and epileptic seizures (Koussa et al., 2006) have been linked to vardenafil, and transient global amnesia to tadalafil (Schiefer and Sparing, 2005), both sildenafil-related drugs.
